Founded in 1987, the Yale-New Haven Community Medical Group includes over 700 Primary Care Physicians and Specialists in over 200 practices.

Organization structure
The Yale-New Haven Community Medical Group is a taxable, not-for-profit membership corporation governed by an elected Board of Directors that is responsible for the governance and management of the company. The corporation contracts with nine health plans operating in Connecticut, serving approx. 150,000 covered lives.
As Physician leaders, we have committed to a Clinical Integration Model that fosters, measures and recognizes Physician and Patient behaviors that are indicators of performance improvement and predictors of positive clinical outcomes. All members of the Yale-New Haven Community Medical Group are required to comply with approved best practice programs, protocols, policies and procedures found in evidence-based medicine and developed through consensus among our network clinical experts and leaders.
The Yale-New Haven Community Medical Group works in clinical partnership with the Yale-New Haven Hospital, the Yale Medical Group, health plans and local, regional and national agencies to implement population-based disease management programs to achieve full clinical service integration and quality outcomes.
