The Yale-New Haven Community Medical Group reorganized for the purposes of clinical integration in November 2003. Our bylaws were revised to reflect the new requirements.

Members

Every member physician is required to sign a Participating Provider Agreement and go through a credentialing process for new members or recredentialing of existing members. In addition, members are subject to Professional Liability & Adverse Actions Guidelines and Membership Appeals and Physician Termination & Suspension Policy. Members' access to their files is governed by the Applicant/Members Access to Files & Correction of Information Policy. All policies are subject to review and oversight by the YNHCMG/YNHH Credentialing Committee, and approval of the Board of Directors. Members of the Board of Directors, the Credentialing Committee, and others involved in the credentialing process are required to sign a confidentiality and non-discrimination agreement.
